[For facilities] About report at the time of outbreak of infectious disease
If a mass infection of infectious diseases occurs at facilities, etc., it is necessary to determine the diagnosis, provide appropriate treatment, check the route of infection, and prevent the spread of infection. Please pay close attention to infection prevention on a daily basis, and if there is an outbreak of an infectious disease, please check the report destination at the bottom of the page and report it in the report format.

The survey will be conducted based on the flowchart shown above.
2.Reporting Standards
①When two or more fatal or serious patients occur within one week due to the same infectious disease or food poisoning.
②When 10 or more patients with the same infectious disease or food poisoning or suspected of them occur or more than half of all users
③Even if it does not fall under 1 and 2 above, it is suspected that an outbreak of infectious disease etc. exceeding the normal outbreak trend, especially when the facility manager deems it necessary to report.
※It does not have a diagnostic name. If you have confirmed multiple similar symptoms, please contact us as soon as possible.
3.Reporting Form
(1) When infectious gastroenteritis is suspected
Documents to be submitted
・Facility sketch (a simple handwritten or facility brochure is acceptable)
The above line list includes tabs for the elderly and disabled and those for elementary school student facilities.
It does not have to be attached Excel, but please submit a table that shows the change of the outbreak patient to the ward health center.

①Please report the line list and a sketch of the two facilities by e-mail (is-teiki@city.yokohama.jp).

(2) When suspected of influence, COVID-19, RS virus, etc.

・Line list of influenza, corona, RS virus, etc. (Excel: 63KB)
・ Facility sketch (a simple handwritten or facility brochure is acceptable)
The above line list includes tabs for the elderly and disabled and those for elementary school student facilities.
It does not have to be attached Excel, but please submit a table that shows the change of the outbreak patient to the ward health center.

(3) When infection is occurring at a medical institution

If the infection spreads within a medical institution, please use the "Medical Institution Line List".
