About license application of medical personnel

License issued by the Minister of Health, Labor and Welfare (doctors, dentists, pharmacists, public health nurses, midwives, nurses, dietitians, clinical laboratory technicians, sanitary laboratory technicians, medical radiologists, physiotherapists, occupational therapists and autopsy qualification certification application), Kanagawa prefectural governor's license (associate nurses, former public health nurses, former midwifes, etc.)

In principle, when applying for a re-grant, in order to prevent false applications by malicious third parties, a certificate that can identify you, such as a driver's license, health insurance card, etc. Please bring your own and come to the agency (medical-related occupation excluding cooks and drug dealer licenses). If it is difficult to apply by the person, please prepare a power of attorney, bring a certificate that can identify the identity of the assignee, and come to the Agency. In the case of power of attorney, please fill in the Address and name for both the power of attorney and the power of attorney.

If you wish to receive (receive) your license by mail, we will only handle your application (in the case of proxy/agent application, we will not accept your application). Please bring a certificate that can identify you, such as a driver's license or health insurance card, and come to the agency. In this case, stamps, envelopes, and cardboard (to prevent folding) related to mailing will be borne by the applicant.
