　home helper (home-care worker) dispatches home helper (home-care worker) to support the daily lives of people with severe physical disabilities (children), mentally ill persons, and those with incurable disease (359 diseases specified by the government).
In addition, we perform on-site bathing or facility bathing service for severely disabled person who has difficulty bathing at home. (The long-term care insurance system takes precedence.)

　We provide and lend daily life tools to those with severe physical and intellectual disabilities and those with incurable disease (359 diseases specified by the country).

　In addition, we issue assistive device to person having certificate of the physically disabled as needed.

　Person having disability certificate of hearing impairment is targeted for assistance. A medical certificate form for obtaining a notebook will beElderly and Disabled Support Division.

　Those who have a certificate of the physically disabled who are 18 years old or older can receive medical care at designated medical institutions such as the country to reduce disability and restore functions. (There is a self-pay depending on the municipal tax amount of the household.)

If you present your Medical Certificate along with your health insurance card at the medical institution, you will not have to pay a portion of the insurance medical treatment fee.
If you are not able to present Medical Certificate for a severely disabled person for unavoidable reasons, or if you go to a hospital that does not handle this system or a hospital outside the prefecture, you can get a refund if you go through the procedure at the ward office health pension section of your ward.
In addition, if you are consulted at a hospital outside the prefecture for those who are eligible for the longevity medical system (late-stage elderly medical care system), you will be automatically transferred to your designated account at a later date.

Yokohama-shi home disability allowance was abolished on April 1, 2010.

　Those who have Address for more than 6 months in the prefecture as of August 1 (base date), and fall under the following disability requirements. However, those who have entered (hospital) facilities or hospitals for more than three months continuously for one year until the day before the base date (from August 1 of the previous year to July 31 this year) are not eligible . 1.Those who fall under multiple disabilities

　B. Nursing notebook (love notebook) A person who is judged to be in the state of A1 or A2
　Person who received grant of mental patient health welfare notebook first grade
2.Persons who have received the welfare allowance for children with disabilities or special disability allowance under the provisions of the Act on Payment of Special Child Raising Allowance (excluding those that fall under 1)

This is an allowance for those who have severe disabilities over the age of 20 and who need special care at all times.
(There is an income limit. Excludes those who have been admitted to the facility and those who have been hospitalized for more than 3 months. )

This is an allowance for those who are raising children with disabilities who need assistance and rest under the age of 20.
(There is an income limit. Excludes elementary school studenｔ who enter the facility.)

This is an allowance for those with severe disabilities under the age of 20 who need constant care in their daily lives.

　There is a disability support ordinance that requires guardianship support. Even if a person with a disability cannot receive nursing care from their parents, we provide consultation so that they can live with peace of mind in the community.
