Check ups for Infants and Toddlers

This is an important milestone for children. Don't forget it.

About infant medical examination (4-month-old child, 1-year-old 6-month-old child, 3-year-old child) carried out in ward office

 We will send individual notices ("Notice of medical examination") to the target person approximately 3 to 7 weeks before the medical examination date.
 The date of the medical examination is a guide allocated according to the date of birth. The consultation schedule may be changed, such as when the number of eligible persons increases, so be sure to refer to the “Notice of Medical Examination” sent individually.
 We apologize for the inconvenience of inhabitants of a ward, but we ask for your understanding and cooperation.
 If you have any concerns or concerns about your child's development, development, or childcare, please contact Asahi Ward Office Children and Families Support Division (954-6150).

Notes

  • If it is inconvenient on the date of the medical examination described in the “Notice of Medical Examination”, please come on the next and subsequent medical examination days.
  • If your child or companion has symptoms such as fever or cough, please postpone the medical examination. ※If you are over 37.5 degrees, you cannot enter the medical examination site.
  • Please wash your hands before the reception.
  • Please take your used diapers home with you.
  • Please carry your luggage when measuring or consulting, so please prepare a bag on the floor as compact as possible.

Notes

※1 Spread it on the bed at the time of consultation.
※2 Use it as a portable bag for storing your child's clothing, etc. during measurement and consultation.
※3 Use the toothbrush when individual guidance is required.
※4 The results of the urinalysis will be announced by mail at a later date.
※5 Please bring it in the enclosed container. If you can't collect urine on the day, you don't need to bring it with you. In addition, examination is not possible even if only urine is sent.
