Community Care Plaza is a Yokohama City's unique facility that engages in various initiatives as a familiar welfare and health base so that everyone, including the elderly, children, and people with disabilities, can live in the community with peace of mind. As of April 2023, there are 145 places in the city.

This service is based on the long-term care insurance system. Care managers create a care plan for the use of nursing care services so that users can live independently at home as much as possible, and work with businesses and related organizations so that they can provide appropriate services based on the plan. This is a service provided by the long-term care insurance system. We provide users with services such as daily life support such as meals and bathing, as well as functional training to improve living functions on a day trip.
※There are also community care plazas that are not implemented. In addition, there is a community care plaza that provides disability welfare services.
