The system has changed since May 8, 2023.
From May 8, 2023, the position of the Infectious Diseases Control Law for COVID-19 has changed, and there are no restrictions on actions based on the Act.

To the facility manager
If a new coronavirus infection (including suspicion) occurs at a medical institution or a facility for the elderly or disabled, and the following conditions apply, please fill out required items in the “Infection Status Report Form for Facilities for the Elderly and Disabled” and submit it to Health Promotion Section by e-mail or fax.
Where a report is required
- When two or more fatalities or serious patients suspected of having COVID-19 infection occur within one week
- When 10 or more patients (including suspicions) with COVID-19 infection or more than half of all users
- Even if it does not correspond to 1 or 2 above, it is suspected that an outbreak of infectious disease etc. exceeding the normal outbreak trend, especially when the facility manager deems it necessary to report.

When submitting by e-mail, please make the subject "[Name of the facility] COVID-19 infection status report".
After confirming the contents of the sheet at Kohoku Ward Health and Welfare Division, we will call the facility and check the status of additional measures if necessary.
