Medical Certificate for infant (birth) mail grant application

In Midori Ward, even mail accepts grant application of Medical Certificate for infant (birth) targeting at the following people.

To be able to make grant application into by mail

◆Person who there is resident certificate in Midori Ward, and takes out social insurance (employee health insurance except Yokohama-shi National Health Insurance)

※We send Medical Certificate for infant to home in ordinary mail after application was accepted

(person taking out Yokohama-shi National Health Insurance issues Medical Certificate for infant by the second floor 25th window (National Health Insurance Section payment responsible) after filing for participation of child who is the second floor of the ward office 26th window (National Health Insurance Section))

Method of mail application

When birth registration form was submitted, please stop at the second floor of the ward office 25th window (National Health Insurance Section payment responsible). It is necessary to confirm the employee health insurance participation situation of child for application. When the name of child is not listed in health insurance card, we cannot issue Medical Certificate on that day. We hand application and, about medical expenses before Medical Certificate issuance, explain system.
Please download whether you receive application at the second floor of the ward office 25th window (National Health Insurance Section payment responsible).　※Please print application at A4 size. In addition, you refer to entry example, and you fill in required items, and please seal

Please send copy of health insurance card which application and the name of child were listed in with sealed letter.

About copy of health insurance card

Attention

When the items mentioned have lack and seal leak, lack of attachment, please be careful as application is not accepted. In addition, self-addressed envelope is not necessary.
