40-74 years old who have taken out National Health Insurance

For those aged 40 to 74 who have continued to take out National Health Insurance before April 1, you will receive a consultation ticket and medical questionnaire required to receive the Specific Health Checkups.
If you are a member of the National Health Insurance after April 2 and wish to see a specific Health Checkups, please contact the ward office Insurance and Pension Division National Health Insurance Section.
About medical institution which can receive specific Health Checkups, please confirm in medical examination conduct institution list (link).

Specific Health Guidance is a three-month support program for those who are at high risk of developing lifestyle-related diseases as a result of specific medical examinations and can expect preventive effects from improving their lives. We will proceed with the program in consultation with specialists such as doctors, public health nurses, and dietitians so that they can be carried out without difficulty in daily life.
As soon as you receive the voucher, please select a specific health guidance implementing agency (link), make a reservation and contact the desired company directly.

• Yokohama National Health Insurance Card (insurance card)
• Use ticket (mailed individually to those who are determined to be subject to specific health guidance)
• Results of specific medical examinations for the relevant fiscal year (medical examination results issued by the medical institution that received the specific medical examination)
