Summary

Acceptance certificate proves that report of family register was accepted.
Please request from ward office which you reported. We do not handle in other ward offices and service counters in the city hall. Electronic application is not possible. We can request by mail.

When we request by mail

About indication of the days

  • By request by mail, it takes total days of the processing days and the round-trip mail carrier days. You usually anticipate around 10th, but please request with room until certificate arrives after mailing as the days may suffer by postal circumstances and factors such as holidays more.
  • In the case of dispatch, please examine the special delivery mail use in case of sending, return of documents.
  • You have any questions about case and documents and request contents which plural certificates were applied to for at a time, and cases that confirmation needs need the days for certificate making. Please note that you need the days before return than usual.

Necessary thing (documents to send)

  • Bill
  • Copying of identity verification documents which Address was listed in
    Driver's license, my number card (personal number card) or Basic Resident Register card, residence card with photograph or special permanent resident certificate (certificate of alien registration), health insurance card
    • "Copy of mask which full name is listed" in and "copy of mask which Address is listed" in are necessary.
    • As you cannot confirm Address with passport (passport), please send other documents.
  • Expense (fixed-amount money order or money sent by registered mail)
  • Self-addressed envelope
    • You fill in return address Address (Address listed in "copying of identity verification documents which Address was listed" in mentioned above), and please attach stamp.
    • When you request acceptance certificate of fine paper (B4 size), please offer self-addressed envelope which paper of B4 size can enclose.
    • We pay scrupulous attention after confirmation at the time of shipment so that there are not dirts, but acceptance certificate may have fold at the time of delivery by mail. At the time of request, we enclose cardboard and file across paper for B4, and I would like consideration. In addition, please note that you come and cannot take responsibility by postal accidents about loss, damage in mail center.
  • Proxy (only as for the proxy/agent)

Procedure method

You put all the necessary thing (documents to send) mentioned above in the envelope, and please mail Yokohama-shi mail request office work center expectation of request.

About fixed-amount money order
  • Fixed-amount money order hopes that we have you remit fee and the same amount so that change is not paid.
  • Validity of fixed-amount money order is six months from day of issuance. We hope that we have you send thing that there is around one week remaining validity.
