Passport application form

Passport application guidance and application forms are distributed at Public Relations Section (101 window) on the first floor of the Izumi Ward government office.
　
At present, the application form is very short and it is not possible to hand it over to everyone. The application form is scheduled to arrive in early June.
If you are in a hurry, please use the Ministry of Foreign Affairs download application form or cause inconvenience, but please come to the passport center window.
In addition, style of application was changed from March 27, 2023. Please note that old application forms cannot be used. For details, please check the homepage (outside site) of Kanagawa Prefecture.

There are two types, one for a 5-year passport and a 10-year passport.

• Kanagawa Prefecture Passport Center Head Office (outside site)

Those who can apply for a passport in Kanagawa Prefecture are those who have registered as residents in the prefecture.

• Yokohama City Centerminami Passport Center

Those who can apply for a passport at the Yokohama City Centerminami Passport Center are those who have registered as residents in the city.

※You can't apply at the ward office.
※If you live in Yokohama, you can use the convenient one at "Honjo (Yokohama)" or "Centerminami".
※If it is deemed appropriate from the viewpoint of the applicant's convenience, such as going to school or commuting, it is also possible to use the passport center branch office, branch office, or business trip window in the prefecture outside the city.

• Application forms, certificate of family register or abstracts, photos, identification documents, last passport, etc.

Of these, those who have permanent domicile in the city can take a certificate of family register or an abstract at the ward office.
