We sent an e-mail of the survey list to the staff in charge of facility A in the ward where a new coronavirus-positive person occurred from Asahi Ward Health and Welfare Division in order to investigate those who had contacted the positive person. At that time, I accidentally attached a list with personal information for 76 facilities B in the ward was accidentally attached, and six staff members of Facility A could view the list.
In addition, a password was set for the roster, and only two people actually viewed the list. Ward staff goes to facility A, confirms personal computers and confirms deletion of list.
We deeply apologize for any inconvenience this may have caused to all concerned parties.
